You can pay for your office visits and treatment by cash, check, credit or debit card at the time of treatment. We accept Mastercard, Visa & Discover. We also accept most major dental insurances. To discuss payment options or insurance coverage, please call us at 508-832-0919.

We make every attempt to verify your insurance eligibility, however an estimate of benefits is not a guarantee of payment.  Final determination of payment is ultimately determined by your insurance carrier.  If you have questions regarding your account, please contact us.

Patient Information

First Visit

At your first visit Please assist us by providing the following information at the time of your consultation:
1 - Your surgical referral slip.
2 - A list of medications you are presently taking.
3 - If you have medical or dental insurance, please bring the insurance cards. This will allow us to help you process any claims.
4 - If you have any xrays from your dentist, please bring them with you. Sometimes additional xrays need to be taken in our office.
5 - All patients under the age of 18 must be accompanied by a parent or guardian.

6 - For new patients, you can print, read, fill and sign the History and Physical form, HIPPA Notice, and Privacy Notice before your visit to the office.
